Default bg is high with insulin

My bg is never below 200 range, and I take 60 units of lantus, at 6:00 pm each day, along with 10 units of humalog, before each meal, also I take a actos 45mg tab each day! That's just the meds for my diabetes! I am a 48 year old male, that was diagnosed with diabetes, when I was 38, but only on insulin, for last two years. I have a regular doctor, that just keeps uping the dosage, he finally set me up with a specialist, on January 10th, maybe he can give me some real answers, thanks for letting me vent!!
