Default Out of Control Type 2

Hi all! I'm hoping you could maybe give me some insight. I was diagnosed a year ago with Type 2. Ever since then, I've been working with my Dr to try and get my sugars under control. My A1C keeps climbing from month to month (up to a 10.4) and my sugars average in the 300s (today before breakfast 353). I'm on metformin, glyburide, januvia and I just started insulin today. Why won't it come down? I go to the gym and I watch my intake. I'm worried that if the insulin "works" that I will just fall from a 300 to around a 100 and just feel awful. Thoughts? Thanks!
