I think there is a better explanation. Caffeine is a stimulant that jogs the sympathetic nervous system. That causes a rise in blood glucose levels. Same thing is seen with asthmatics that use an albuterol inhaler. Any stimulant drug will do the same thing (caffeine, albuterol, amphetamines, theophylline, etc.)
The amount of sugars in the small amount of milk used as a creamer in a cup of coffee would not be expected to affect BG much.
