I totally get it and have been there myself (we sound a lot alike). In part I think it's denial and a lack of motivation to be "different". Sometimes I find that I'm just too busy to be diabetic - single working mom etc. I often dread BG testing because it's like the number tells you that you Failed or Passed and I hate that.
But you have to find your motivation. For some it might be the friendly competition with a friend, for me it's being there for my kids, for you it will be something unique. What do you love to do? Imagine not doing that because you were too sick and that is probably the place your motivation will come from.
Just don't beat yourself up!!! Get back on the Diabetic horse and start riding it again. I believe in you!
