The 21 lbs. weight loss is a great start and I am sure that you BGL (Blood Glucose Level) has shown this. It would appear that the biggest problem that you have to overcome is the fasting BGL. You could try a thirty (30') minute walk after dinner / before bedtime, I am sure that would help some. If you have to go to insulin, you will find that it is not that bad. It is more of an in-convenience that anything. Good luck, I hope that you can escape the insulin.
