If you set your email address in the My Info; Emails the App should already know your email address. However, I have my iPhone set to CC myself, under Settings; Mail; Show TO (ON) and Always BCC Myself (ON). This information is shown during, as you describe, when the "TO" line comes up blank, so does mine. I suppose that you could input your doctors email address at this time. Truthfully, I don't really know how it works - it just works for me....
