The Dexcom G4 (CGM) is an excellent choice. I assume that you saw the different forms on the website, just make sure that you are a candidate for the CGM .... documented lows during the night and such.
The pump info I can't help with. I would think that since you were previously on the pump, the insurance should not have a problem. I think that I would call the Endo first and see what he/she has to say and then call or contact the CGM and pump companies. Normally they will take care of it all, but the doctor will have to prescribe both.
